Hello, I am using windows 7, the in game GM's haven't been able to help me at all, neither has the program dxwnd. I recently joined the server, and haven't been able to use windowed mode (Alt + Enter ) at all. And yes, I have tried using system options > tick the fullscreen box. Thanks in advance.

if alt + tab works, then it might be an issue with your ATI driver. Try to roll it back one version

Might as well leave this here. Whenever I start MS it always starts in full screen, no matter my settings from the last visit. If I had the game windowed when I quit, it will still start in full screen the next time I start the game.
